I've learned a few things about accessing a physical CD with Mednafen on Mac OS X.
I posted a bug report for libcdio and corresponded with one of the developers. To sum up the results, libcdio's MMC commands don't work on Mac OS X. This is a known issue and there's no good solution to the problem. He called MMC access on OS X a "black art."
A workaround for Mac Mednafen users is to use libcdio block read commands to extract the data from a physical CD. An example of the process can be seen with "cd-info --iso9660" from the command line.
The developer also mentioned how to get more info from the libcdio library at run-time:
Another workaround would be to use a library capable of reading CDs on Mac. I know that VLC Media Player has no issue accessing CDs on Mac, but I don't know what library (if any) it uses for reading CDs.
Neither workaround is ideal and I don't have the programming skill to implement either one. I'm not expecting you or anyone else to fix/workaround this Mac only issue. I'm just passing along what I've learned.
I'm grateful that Mednafen works as well as it does on a platform you don't officially support.

I'm not having that issue exactly, but I have reproduced a similar issue, and I'm looking into a fix for it.
