I'm reading the internal conversion to RGB is lacking in precision and could be missing some colours.
I wonder if this problem is real and how many games it impacts.

So it's more about the game developers palette choices done using the standard composite output, now getting screwed up when displaying with RGB.
Some tints that were different ends up being so close they become hardly distinguishable.
That can be fixed with a custom palette that mednafen supports.
